Migliaccio & Rathod LLP is currently investigating Florida Orthopaedic Institute (FOI) for failure to adequately safeguard patients’ Protected Health Information (PHI), resulting in a data breach. The medical group recently announced that, on April 9th, they were the victims of a ransomware attack that that resulted in the encryption of data stored on their servers. Subsequent investigations revealed that patients’ PHI may have been stolen during the incident. Compromised information includes:
- Social Security Numbers
- Medical Information (including diagnosis codes)
- Insurance Plan Identification Numbers
- Payer Identification Numbers
- Payment Amounts
- Insurance Claims Address Information
FOI has alerted those potentially impacted via mailed letter and is offering them complimentary credit monitoring services. The deadline to enroll for these services is September 30th.
