Choice Specialty Pharmacy recently reported this data breach to the United States Department of Health and Human Services. The pharmacy discovered that an unauthorized third-party gained access to important files containing the private information of patients. Subsequently, Choice Specialty Pharmacy conducted an investigation that uncovered a cybercriminal had accessed the affected accounts containing the personally identifiable information and private health information of certain patients and employees. While Choice Specialty Pharmacy , the compromised information potentially includes the following sensitive data:
- Full Names
- Physical Addresses
- Dates of Birth
- Social Security Numbers
- Medical Record Numbers
- Driver’s License Numbers
- Health Insurance Information (including Account Numbers and/or Policy Numbers

All of the information stolen is likely valuable and dangerous to affected victims.
