Migliaccio & Rathod LLP is currently investigating Team Alvarez Insurance Services, a Santa Ana insurance brokerage firm that manages employee and other healthcare plans, for failing to safeguard sensitive plan patient information in a data breach that occurred in August of 2021. The data breach was caused by unauthorized access to the firm’s network that contained the personal information of health benefits plan participants. Examples of the types of data exposed in the leak include names, dates of birth, addresses, social security numbers, medical records, lab results, diagnoses, medications, health insurance information, and dates of services.
